COVID Registration Waiver

By way of completing the NASIG 2024 Annual Conference registration form:

• I acknowledge and understand the continuing public health challenge posed by the contagious nature of the Coronavirus/COVID-19 and other infectious respiratory diseases. 
• I further acknowledge and understand that by attending the NASIG 2024 Annual Conference I am increasing my risk to exposure to the Coronavirus/COVID-19 and other infections respiratory diseases and that NASIG cannot guarantee that I will not become infected.
• I agree to comply with all set procedures to reduce the spread of Coronavirus/COVID-19 and other infectious respiratory diseases while attending the conference and all conference events.
• I further acknowledge and understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 and other infectious respiratory diseases may result from the actions, omissions, or negligence of myself and others, including, but not limited to, hotel staff, and other hotel guests.
• I hereby release and agree to hold NASIG harmless from, and waive on behalf of myself, my heirs, and any personal representatives, any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of NASIG, or that may otherwise arise in any way in connection to the NASIG 2024 Annual Conference. I understand that this release discharges NASIG from any liability or claim that I, my dependents, my heirs, or any personal representatives may have against the organization with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to NASIG or the NASIG 2024 Annual Conference. This liability waiver and release extends to all NASIG officers and volunteers.
